What this website does with information — in plain language, and briefly, because it does very little.
What this site collects
Nothing automatically, on almost every page. This website is a set of static pages. It does not set advertising cookies, it does not build a profile of you, and it does not sell or share anything about your visit.
The exception is the payment-plan estimator, and it is described in full below.
The payment-plan estimator
Our payment plans page carries a financing tool made by Cherry Technologies, Inc., and some other pages carry a small button that opens the same tool. It is the only piece of this website that is not ours, and it is the only piece that involves another company while you are simply reading.
On the pages that carry it, your browser loads the tool from Cherry and from their analytics provider, Segment. Cherry can therefore see that a browser opened one of those pages, along with the ordinary things any web request carries — your IP address, your browser, and the page you were on. It stores two random identifiers in your browser so it recognises the same browser if it comes back. It does not set cookies, and it is not given your name, your email address, or anything about your treatment.
Nothing is sent to Cherry about you personally unless you choose to apply, and applying happens on Cherry's own site under their privacy policy, not ours. If you would rather not load their tool at all, the rest of the site works normally without visiting those pages, and you can always ask us about payment plans by calling the office instead.
The appointment form
The appointment request form sends what you type to our appointment system so the practice can call you back. That is your name, the phone number and email address you give us, the reason for your visit, your preferred times, and anything you write in the message box. We keep it so we can follow up on your request and so we have a record of it. We do not sell it and we do not use it for advertising. If your email app opens instead, it means the form could not confirm your request reached us. Usually that is because it genuinely did not go through, and the email is what reaches us. On the rare occasion the request had already arrived when this happens, your email becomes a backup copy rather than a duplicate we act on twice, so please still send it.
That fallback email, on the occasions it is needed, is handled the same way as any other message to the practice: read by our staff, used to contact you and schedule your visit, and retained with your patient file if you become a patient. A request that reaches our appointment system through the form is handled the same way, without the email step.
Please do not send detailed medical information by email or through this form. Email is not a secure channel. Tell us what brings you in, and we will take the details in the office or over the phone.
Your dental records
Health information we hold as your dental provider is governed by HIPAA and by our Notice of Privacy Practices, which is a separate document available at the office. It is not reproduced here. To request a copy, to ask what we hold about you, or to request a correction, call 770-629-9201 or write to us at the address below.
Links to other sites
Some pages link out — to Google Maps for directions, for example. Those services have their own privacy practices, and this policy does not cover them.
Changes
If this policy changes, the date at the top of this page changes with it. Material changes will be described on this page rather than made silently.
